Forms and management plans
- Student Health Support Plan – this must be developed by the school in consultation with the parents/carers and where appropriate the student’s treating medical team
- Medication Authority Form – this form details the medications required during regular school hours, the dose, administration and storage
- Diabetes Action and Management – diabetes Victoria contains links to both the diabetes management plans and the diabetes action plan which are prepared by the student’s treating medical team and provided to schools by parents or carers
