Forms and management plans
- Student Health Support Plan (DOCX)(opens in a new window) – this must be developed by the school in consultation with the parents/carers and where appropriate the student’s medical treatment team
- Medication Authority Form (DOCX)(opens in a new window) – this form details the medications required during regular school hours, the dose, administration and storage
- Diabetes Action and Management Plans(opens in a new window) – Diabetes Victoria contains a link to the diabetes action and management plan which is prepared by the student’s medical treatment team and provided to schools by parents or carers

- Diabetes Day for Educators(opens in a new window) – a paid, full day, in-person program designed to increase staff confidence to safely support students living with type 1 diabetes at their schools. Please note that this program is not part of the Diabetes in Schools Program. School staff who complete the Diabetes Day for Educators must still complete the Diabetes in Schools Program
