- Primary School Nursing Program brochure – printable brochure for parents/carers with information about the initiative, including privacy information and key contacts
- Primary School Nursing referral form – this form can be used for referring students in grades 1 to 6 for a health assessment as part of the Primary School Nursing Program. The referral can be initiated by either a teacher or parent/carer. The form must be completed by both the teacher and parent/carer before the referral can proceed
